1. My colleague Namin had a simple operation, so she took the afternoon off. As her number is used other departments of my university, always ringing many calls from foreigners. I had to response to the call for her number as well this afternoon. Answering the phone in English is interesting 😀

 

2. I dropped by Watsons on the way home from work. An cosmetic product that I needed had an promotion event 1+1. I picked the only one remained up and paid.

 

3. Today only three people was in the English conversation class. Naturally I got more chances to talk about the topic that my teacher suggested. We discussed effects of advertisement.

Doctors need to ensure relatives ‘are fully-informed’

In this week’s Scrubbing Up opinion column, Prof Mayur Lakhani chair of the Dying Matters Coalition, urges doctors to be more open and frank about preparing patients and their families for the end of life.

Imagine a situation where most people with a common condition are undiagnosed and where opportunities are repeatedly missed to identify the problem and to offer good care.

What is this condition? It’s dying.

Each year, an estimated 92,000 people in England are believed to need end of life care but not receive it.

As a practising GP I have seen distressed relatives after a patient has died in hospital.

Often they have not had a chance to see their relative before they died and were unaware of the seriousness of the condition, despite repeated admissions with deteriorating conditions.

Many such patients are never formally identified as at risk of dying and not assessed for end of life care.

One relative said something that haunts me to this day: “I wish the doctors had told me that my mother was dying.”

As a result too many people still die in distress with uncontrolled symptoms, or have futile interventions when this will not make any difference.

All of us, including doctors, must do more to talk about dying.

‘Too late’

A new ComRes research for the Dying Matters Coalition confirms there is a widespread reluctance to talk about dying and death.

That is why we want to encourage people to talk more openly about dying and bereavement and to make plans around end-of-life care.

Most people have not made a plan of their choices and wishes in the event of a serious illness.

“Be honest and don’t offer false hope”

People often think there is plenty of time when the situation arises but often this is not the case and it can be too late.

Doctors are trained to cure, but as people live with long-term conditions, the medical profession needs to change how it works with dying patients.

We need to bring dying back into people’s homes, rather than in hospital, care homes or hospices.

As increasing numbers of people develop and live with dementia, it is vital that doctors talk about the kind of end-of-life care people would choose, while people are healthy.

There’s a huge change required in our culture, starting with the medical profession.

Having learnt a lot from the Canadian communications expert Prof Stephen Workman, I would urge doctors to do three things.

First, be compassionate but direct when talking about dying.

Doctors speaking to relatives about an ill patient who is giving cause for concern, should not just say “your husband is seriously ill”, but should add “It is possible that he could die”. This allows all involved to prepare.

Secondly, be honest and don’t offer false hope.

Think twice about offering a false choice of procedures like cardiopulmonary resuscitation (CPR) when someone is actually dying and chances of recovery are minuscule.

Instead offer supportive care to keep the patient comfortable and allow them to die a natural dignified death.

And, thirdly, when doctors diagnose a serious illness they should ask the patient whether they would like to talk about what they can expect and what is likely to happen.

Most people die from frailty in old age, and will be well known to health and social care services.

Importantly, this gives opportunities for talking about end of life wishes and advance care planning.

My ambition is to reduce the fear of dying and increase knowledge and awareness of palliative care.

It is vitally important that every person who is coming to the end of their life is spotted early enough and supported to make a good advance care plan.

I know just how hard it is to address issues like this in the frenzy of busy surgeries and wards.

But we as doctors need to up our game.

All of us, especially doctors can play a part in ensuring everyone is able to have a good death.

Comment : Yesterday I watched a TV program that an old-aged Korean actress appeared. She talked about “well dying”. When she was 40’s, she never thought about death. Dying at 70 is the best was her thought. However, she is now 67 and she no longer think that she will die in about three years without concerns about her sons and works. After 50’s she get to think about her death seriously.

Just in good time my English teacher assigned homework : Read an article for discussion. I find the right one at BBC website. 🙂

1. I’ve had a cold since yesterday. Today much better than yesterday, but I couldn’t go to work at morning. After 1:00PM I went to the office though I still thought I could die. However, everyone in the office talked to me with concerns.

2. Summer is just around the corner. Suddenly, the weather became so hot. My English teacher and Students went to the cafe in the University. We spend time drinking tea on the terrace.

 

3.Last week I ordered some clothes at a cheap price on online shopping mall. They were duly received 🙂

[3 happiness-2nd May]new friend/insurance money/confidence

 

1. I made a mew friend a few days ago. He is from China. I cannot speak any word in Chinese, but we are now friend on Facebook. Today he came our office and say hello to me. We even communicated via messenger 🙂 Wanna build stronger relationship with him.

 

2. In 2001 I was taken to the emergency room by rescuers. I has procrastinated to ask insurance money to an insurance company. Finally I applied it the day before yesterday and today the insurance money was transferred to my bank account. hehe

 

3. These days I was feel low because I lose confidence about speaking in English. Today I tried to talk a lot in my English conversation class, so much feel better. 😀

1. What a beautiful weather! I and my colleagues was going to eat lunch open in the air. We picked up the Potato Gold Pizza. I chose the menu hehe… It was really yummy and reasonable price. I was happy about finding great restaurant.

2. I talked on the phone with my bf. We fixed an situation and had role play over the phone. it was really fun. lol

 

3. Mayday makes a day without English conversation course. I decided to watch a movie, but after finishing my work, rain was starting. I just spend time watching dramas on my bed. It was not bad.

These days I really stressed out and tired 😦 do not want to do anything. I don’t know why just guessing because of a heavy lead of work. I’m trying to, but getting harder and harder to find daily luck.

1. The rain has stopped. The alarm makes me wake up early morning at 7:00. In fact, I made up my mind to go jogging before preparing daily life. However, the warm electric blanket made me paralyzed haha… Finally I came out from my blanket at 7:30. 30 minute earlier than other days. Not bad 🙂 I stretched my body in remaining time.

2. I found that an online shopping mall that sell many kinds of earrings very cheap! I ordered four ones, but it was just 2$! Amazing price.

3. Sometimes I release my stress by painting my nails. It gave me pleasure when I seeing polished nails.

1. I arranged documents with Excel without boss’s order for dealing with a matter of an Invitation Letter easily. Just at a good time my boss required me  to arrange an Excel file.! 😀 Also, some students did not get a concept of exchange student and mis-type their information in an application form. I modified their status few days ago, so can reply to the director’s email promptly today.

2. I had chopped roasted chicken and roast pork for dinner! These food is Usually these two kinds of food are main course.

1. I’m working at Office of International Affairs in Korea University. It is located on 3rd floor of new building which name is Dongwon Global Leadership Hall. I like its location because it has good facilities, especially there is rest-room for female students on 1st floor.

Today I was supertired and dying to sleep, so I went down and took a nap on Ondol. After taking a nap, I had a cup of coffee and it really helps to refresh myself.

2. Rainy day always makes me want to drink 😀 hehe… My college and I had Green Onion Pancake and Makgeolli (Korean traditional alcoholic drink)

3. We went to Karaoke! It’s been a long time. I can relieve all my stress by singing a song 😀 I was a little tired, but I’d say I had a great day.

1.I forgot to bring by wallet today. My colleague treated me for lunch. Actually, I was purposed to go “Namsan” to enjoy seeing cherry blossom. Therefore, the fact makes me frustrated because I can go nowhere without my wallet. Fortunately, there was a parcel in my office to send during business hour, so on the way to post office I dropped by my room and brought my wallet.

2.It was my first visit to Namsan, so I did not know the route. I was on trail and took a guess the right way. Indeed, I was a good fortune-teller! Eventually it took me my final destination ‘Myeongdong’ after walking with awesome cherry blossom trees.

3.On arriving Myeongdong subway station, there was music playing which is by 10cm. These days I practice guitar playing when I have free time and the most lovable song is the song by 10cm. Moreover, the next song was ‘don’t you know’ by No Reply which contains my precious memories. It was BGM of a video which I made for my bf on an anniversary. I was just listening songs standing and seeing Namsan Tower during 10 minutes.
